Jackson School District HIB 338 Reporting Form
The HIB 338 Form provides a means for a parent, guardian, or school staff member to report an incident of harassment, intimidation, or bullying in a confidential manner.
Upon receiving the form, the principal or the principal’s designee shall implement the Jackson School District's policy and procedure for allegations of HIB.
