Are You the Parent of a Child Entering 6th GRADE or Pre-School this September?

State law requires students in these grades have certain immunizations (Diptheria, Tetanus, Pertussis (Tdap) and Meningococcal, depending on grade).

See the letter and immunization update form you can bring with you to your doctor.

This update form can be returned to your school nurse anytime.

Please note that it must be returned to your school nurse by Oct. 13, 2023 or your child will not be allowed to attend school after that date.

NOTE: Preschool students have until Dec. 31 to receive the influenza immunization.
