Once registration opens, there are TWO WAYS you can fill out the registration forms (See Below). Our online pre-registration system streamlines the process and will save you time during your kindergarten registration appointment.
Regardless of which way you choose, you will still need to come to a registration appointment in person with your child to submit your documents and proofs of residency. Any of the documents you see below can be downloaded from this site or picked up at any elementary school.
Requirements for students who attend a Jackson School District Pre-K program (either Pre-School Disabled or our tuition-based Pre-School Inclusion program):
You do not need to fill out the online registration forms that you completed when you registered for Pre-School. However, you DO need to call and make a registration appointment for February or March and bring your child to Kindergarten Registration to:
- Submit a new physical examination form;
- Submit a new dental examination form;
- Show proof of 5-year-old immunizations (if your child is not yet five, you will submit these after they reach the age of 5);
- Re-submit four proofs of residency for review.

Note: You can use the forms below if you are registering for either the current year, or the next school year.

Option 1: Online Pre-Registration

STEP 1:
Complete the Online Pre-Registration Form. Please be sure to use the CORRECT SCHOOL YEAR from the drop-down menu when registering.
STEP 2:
Download, print and complete the Kindergarten Supplemental Forms in English or Kindergarten Supplemental Forms in Spanish.
STEP 3:
Call the school registrar to make an appointment. Bring your printed copy of your Online Pre-Registration Forms, the completed Supplemental Registration Forms and proofs of residency to your elementary school to your registration appointment.
Option 2: Traditional Registration Forms

STEP 1:
Download, print and complete the full Kindergarten Registration Packet in English or the full Kindergarten Registration Packet in Spanish.
These forms are also available for pickup in our elementary schools.
STEP 2:
Call the school registrar to make an appointment. Bring your completed Registration Forms and proofs of residency to your elementary school to your registration appointment.
Age Requirements

In order to be registered for Kindergarten, children must be five years of age on or before October 1 of a given school year. There are no exceptions to this age requirement. Only an original Birth Certificate with a raised seal or a valid passport are accepted as proof of age. Baptismal or hospital certificates are not accepted.
