FREE SPORTS PHYSICALS

Sports Physicals for all 5th, 6th, 7th, and 8th  grade students

The Jackson School District is offering a limited number of spots for students who anticipate playing ANY SPORT for the 24-25 school year.   We strongly encourage you to take advantage of these free physicals offered by the district.

You will use this form to reserve your appointment time.

Physicals will be held Monday, June 3rd at Liberty HS from 2:30 p.m. to 7 p.m. and Tuesday, June 11th at Memorial HS from 2:30 p.m. to 7 p.m.

NOTE: You will need to be logged in to your CHILD'S JACKSON SCHOOL DISTRICT GOOGLE ACCOUNT in order to complete this form.

Parents MUST accompany their child to the physical.
