To notify the main office of a dismissal change for your child, please print and fill out the following document:
Can't download or print? No problem. Copies of the letter can be sent home for your use. Simply complete the form with your child's FULL NAME and date and type of dismissal change, then send it to school with your student.
If you send in a handwritten letter, please write legibly and be sure to include your child's full name, the date, and teacher's name. This letter must be separate from any other correspondence intended for the teacher as these dismissal notes are sent to the main office.
